Research Paper survey help

Hi everyone. I'm doing a research paper right now and I was wondering if you guys could help out. All I need is for you guys to feel out a survey. So just think of it as one of those stupid myspace surveys to pass the time. Now of course I'm obligated to say that you can decide to get out of the research at any point that feel too uncomfortable and do not have to feel out all the information. Also your name will not be put on record and none of your personal info will be given out. Instead each person that decides to send me the finished survey; their identity will be replaced with a random i.d. code which I will use when describing your answers in my paper.
Now you can either send the finished survey through myspace or if you don't want to do that you can send it to me at my email:crsnsv16@aol.com

Here's the survey. Some questions will be general while others may need an explanation. If you want to explain yourselves further. Go ahead. Also I need them submitted at the latest by this wednesday at 5:30 p.m. Thank you

1) Age:

2)Gender:

3)Ethnicity:

4)What do you know about Restaurant Etiquette? (proper tipping, eating, appropriate attire, etc,.)

5)Who taught you?

6)How were you taught?(punished then told, taught and then punished when doing wrong, etc.)

7)How often do you go to restaurants?(including fast food)

8)Out of these times, how often is it through a drive-thru or to-go?

9)What do you talk about while you're sitting and eating with friends? with family?

10) Are there different types of conversations between the two?

11)On average, how much do you tip?(give percentage)

12)What do you believe are things you should not talk about while in a restaurant?

13)Do you talk about these things while in a restaurant?

14)Do you talk on your cell phone in restaurants?

15)Do you know the difference between the different utensils? (soup spoon/dessert spoon , dinner fork/salad fork)

16)Have you ever worked at a restaurant? If so, what are the main things that annoy you about your customers?

17)How do you dress when going to a fast-food restaurant? family-oriented restaurant? themed restaurant? fancy restaurant?

18)Do you generally feel good about how you acted while leaving or do you tend to have a degree of guilt?

19)Name the reasons why you go to restaurants (and don't say because you were hungry)

20)Do you believe going to restaurants is more to maintain relations, socialize, make an appearance or is it more about the food

21)Would you say you eat more at home or out? (Do not include cafeteria, dorms, or dining commons in this answer)

22)Do you believe that it matters what fork you use or do you figure: who cares?

23)Do you believe it matters how you dress?

24)Do you think Restaurant Etiquette is important?

25) Do you think Manners are important?

26)Are they the same thing or different in your mind?

27)How often do you make reservations?

28)Do you stick to your reservations?

29)Explain how you calculate out your tips

30)Do you have conversations with those serving you?

31)Do you thank them on your way out?

32)When you decide not to do something because of the consequences is it mostly because you don't want the restaurant to punish you, you will feel shame/guilt/humiliated by your actions, you don't want the people around you to know that side of you, or that you think that it might hurt others and/or you don't want to make them feel uncomfortable? Explain. (Ex:You decide not to get into a food fight because they might ban you. Ex:You decide not to get mad at your waiter for the horrible service because your date is right there)

Thanks again in advance for your help. If you have any comments or suggestions on how to make this better please send them along. and if you would like to know what I'm doing with this info. I'll be able to tell you once I"m done
